This section is focused on securing feedback from our clients, employees, vendors and interfacing agencies from a service perspective. On occasion, we will solicit feedback in an effort to measure our performance against established goals and to ensure that objectives are being met and/or exceeded. Discharge Survey 2008 - This survey is for clients who have decided to leave our program.
Community Feedback 2008 - This survey allows for direct feedback from the community regarding the services we provide.
Client Satisfaction Survey 2008 - This survey allows clients to provide across the board feedback on our level of service and care.
